Authorization to Obtain Consumer Report — FCRA Compliant
This authorization document is provided separately from the employment application, as required by the Fair Credit Reporting Act (FCRA), 15 U.S.C. § 1681b(b)(2)(A). Please review the Background Check Disclosure before signing.
Authorization and Release
I hereby authorize Prime Anchor Point ("Company") and its designated consumer reporting agencies (CRAs) to obtain one or more consumer reports and/or investigative consumer reports about me for employment purposes, including initial hiring decisions and during the course of my employment.
Scope of Authorization
I authorize the Company and its designated CRAs to obtain information about me from any person, institution, or organization, including but not limited to:
Federal, state, and county criminal court records
Sex offender registries (national, state, and local)
Past and current employers for employment verification
Educational institutions for degree and certification verification
Motor vehicle records / Department of Motor Vehicles (where applicable to the position)
Credit reporting agencies (only where permitted by applicable law and relevant to the position)
Social Security Administration / SSN verification services
Global sanctions, terror watchlists, and government exclusion databases
Professional licensing boards and registries
I authorize any person, company, or institution having information about me to release that information to the Company or its designated CRA. I release all persons, companies, and institutions from any liability in connection with the release of information pursuant to this authorization.
Your Rights Under the FCRA
I understand that I have the right to:
Receive a copy of the consumer report if adverse action is taken based on information in the report
Dispute inaccurate or incomplete information in the report directly with the CRA
Request a free copy of my consumer report from the CRA within 60 days of any adverse action
Know the name, address, and telephone number of the CRA that prepared the report
I acknowledge that I have received, read, and understand the separate Background Check Disclosure document and the Summary of Rights under the FCRA.
Duration of Authorization
This authorization shall remain in effect for the duration of my employment with Prime Anchor Point or any of its client companies, and for an additional period as necessary to resolve any pending legal matters, unless I revoke it in writing. Re-authorization is not required for periodic checks conducted during employment.
Voluntary Authorization
I understand that my authorization is voluntary. However, declining to authorize a background check may affect the Company's ability to consider my application for employment or continued employment.
Signature Block (Print & Sign)
This form may be presented electronically or in print. Electronic authorization via our platform is legally equivalent to a wet signature under the E-SIGN Act.
✅ Electronic Consent: When you complete our online onboarding process and check the authorization box, your electronic consent carries the same legal effect as a handwritten signature under the federal E-SIGN Act (15 U.S.C. § 7001 et seq.) and applicable state law.
