Required Disclosure Under the Fair Credit Reporting Act (FCRA)

This document is provided to you separately from any employment application or other document. This disclosure is required by the Fair Credit Reporting Act (FCRA), 15 U.S.C. § 1681 et seq.

Disclosure of Intent to Obtain Consumer Report

Prime Anchor Point ("Company") may obtain one or more consumer reports and/or investigative consumer reports about you in connection with your employment application, and if hired, during the course of your employment. These reports may be obtained at any time after receipt of your authorization and, if you are hired, throughout your employment.

What Is a Consumer Report?

A consumer report is a report prepared by a consumer reporting agency (CRA) that contains information about your personal and credit characteristics, character, general reputation, and lifestyle. The report may include information about:

What Is an Investigative Consumer Report?

An investigative consumer report is a type of consumer report in which information about your character, general reputation, personal characteristics, and mode of living is obtained through personal interviews with neighbors, friends, associates, past employers, or others who may have knowledge concerning the relevant information. If we obtain an investigative consumer report, you have the right to request additional disclosures about the nature and scope of the investigation.

Consumer Reporting Agency

The consumer reporting agency (or agencies) that may be used to prepare these reports include, but are not limited to, background check providers compliant with FCRA and applicable state laws. The name and address of the specific CRA will be provided in your written authorization form.

Your Rights Under the FCRA

Before we take any adverse action (such as not hiring you) based on information in a consumer report, we will provide you with:

You have the right to obtain a free copy of your consumer report from the CRA within 60 days of any adverse action notice, and to dispute the accuracy or completeness of any information in the report.

Authorization Required

We will not obtain any consumer report without your prior written authorization. You may provide that authorization by completing the Background Check Written Consent Form. Your authorization is voluntary; however, your decision not to authorize a background check may affect our ability to consider your application for employment.
